BayCare EasyPass™ (BayCareEasyPass.org, EasyPassForMoms.org) is a membership program that is currently being offered on a limited basis only to qualifying patients of BayCare Medical Group practices and maternity patients at Morton Plant Hospital.
You are responsible for remaining familiar with the Program terms posted on the BayCare EasyPass™ website [BayCareEasyPass.org]. We reserve the right to terminate or change these terms at any time without prior notification. The most current online version of these terms will supersede all previous versions of these terms.
Regardless of how you participate in the Program, benefits can be changed or terminated at any time without notice at the sole discretion of BayCare Health System.
Each member consents to the collection and use of his or her personal and other information and the disclosure of such information to agents of BayCare Health System for the purpose of operating the program, tracking usage of program benefits and the delivery of benefits.
Program Activation and Benefits
BayCare EasyPass™ is a no-cost membership program, no purchase is necessary to participate.
All patients of BayCare Medical Group Primary Care Practices and maternity patients at Morton Plant Hospital are automatically eligible to become members of BayCare EasyPass™, and are eligible to receive its benefits upon activation.
Members can activate their account by going to BayCareEasyPass.org, or EasyPassForMoms.org.
To activate membership in BayCare EasyPass™, simply go to BayCareEasyPass.org or EasyPassForMoms.org website. You will be asked to provide a unique email and to create a password. You are responsible for keeping your contact information up to date; BayCare Health System is not responsible for communications or benefits sent to out of date email addresses.
Only one account permitted per member.
Qualification / Eligibility:
Members must be at least 18 years of age to participate.
BayCare employees and their families are eligible for the Program.
Opt-ins:
By providing your enrollment information you are opting into Program communications and benefits including email, text, direct mail and social postings. If you choose to cancel your membership, you may do so at any time by contacting toll-free (844) 260-7147 or unsubscribing from the email communications through your account profile.
